On 19 Dec 2022, an attending veterinarian contacted Elanco regarding a potential adverse drug experience involving Clean Up II when administered to adult beef cattle. On 16 Dec 2022, a group of approximately 250 beef cows each received 30 ml of Clean-Up II Pour-On Insecticide with IGR (Permethrin, Diflubenzuron), poured onto the back from the withers to tail head for lice control. The cattle also received vaccination with Scour Bos 9 (bovine rotavirus, coronavirus, Killed Virus C perfingens Type C, K99 E coli bacterin toxoid, 2 ml, intramuscularly into the right neck), 12 ml ivermectin subcutaneously into the right neck area, 4 ml Vitamin A and D solution subcutaneously in the right neck area, and a non Elanco multivalent inactive vaccine including infectious bovine rhinotracheitis (IBR), bovine virus diarrhea (BVD), parainfluenza 3 (PI3), bovine respiratory syncytial virus (BRSV), Leptospira canicola, L. grippotyphosa, L. hardjo, L. icterohaemorrhagiae, L. pomona and L. borgpetersenii serovar hardjo (type hardjo-bovis). The cattle were also checked for pregnancy and older cattle had their mouths check for advanced age. All cattle that had a stated broken mouth or only gums were subsequently placed in a cull pen to be sold. The cull pen cattle were estimated to be ages of approximately 10 years and older. The cattle had been fed a ration containing silage due to local drought conditions. Any other concomitant conditions were unknown.

On 16 Dec 2022, one hour after processing, one adult female mixed breed beef cow from the cull pen, of greater than 10 years of age, with broken mouth/ only gums, was found dead and having froth from both nostrils. The attending veterinarian then returned to the farm for cattle examination. The attending veterinarian confirmed information from the dead cow. The attending veterinarian reported the combination of Scour Bos 9 vaccine and Vitamin AD solution was the most likely reason for cattle death, although stated he realized that the cattle received a stated poly pharmacy of medications (received many various medications while being processed). The attending veterinarian stated that Clean Up II had nothing to do in being a part of any cattle signs.

On 16 Dec 2022, an unknown time after application of Clean Up II, 5 of the cull cows, of various ages greater than 10 years of age, of mixed breed, with broken mouths/ only gums experienced increased respirations, increased heart rates, were lethargic, and had signs of anaphylactic shock. 5 additional cows also experienced the same signs. The veterinarian treated the cows with intravenous epinephrine at 1 ml per 100 pounds and dexamethasone at 3 ml per 100 pounds. Treated cattle were found to be improving within one hour. On 17 Dec 22, the 10 cows recovered, no others had been found with issues. The attending veterinarian reported the combination of Scour Bos 9 vaccine and Vitamin AD solution was the most likely reason for anaphylaxis, although stated he realized that the cattle received a stated poly pharmacy of medications (received many various medications while being processed). The attending veterinarian stated that Clean Up II had nothing to do in being a part of any cattle signs.
