5/22/2020 Caller's husband works on a farm, and was working with canola seeds that had been treating with the product. He was dumping bags into the feeder, and running the tractor from 5/17/2020 to 5/19/2020. On 5/19/2020 around 11:00 in the morning he started to experience dizziness, unsteady gait, headache, and became very pale. He was brought to the emergency room at that time where they ran unspecified tests, blood work, and had him on a heart monitor. He was kept in the emergency room for 24 hours to monitor him, and they ruled out stroke and diabetes. He was sent home with a heart monitor for 24 hours. He returned the monitor yesterday, and they did not find any significant issues. They did not talk to the doctor about the product as they just thought about it today. At this time he is still having headaches, occasional dizziness, and has been extremely thirsty. 5/27/2020 Attempted call back to the original caller. Caller reported her husband was dumping the bags of seeds into the feeder and running the tractor. According to the label, to minimize exposure to dust when handling treated seeds, the user should wear coveralls over long-sleeved shirt, long pants, chemical-resistant gloves, head gear, shoes and socks and appropriate respiratory protection (i.e. NIOSH/MSHA/BHSE-approved 9 dust-filtering respirator or fresh air hood). It is unknown what type of personal protection equipment he was wearing. It is also unknown if he has used the product previously in a similar manner and if he experienced signs or symptoms.A temporal relationship exists, he was working with the product for three days and early in the third day started developing symptoms. The patient was pouring bags of the seeds treated with the product into his feeder and running the tractor and may have been exposed by inhaling dust and particulate from the seeds as he pours. He may have also had dermal contact with the seeds. Some dermal absorption does occur with neonicotinoids and toxicity after inhalational exposure has been reported. Clothianidin is a neonicotinoid insecticide which activates the nicotinic receptors. Theoretically, they are less toxic to mammals due to the lower affinity for vertebrate nicotinic receptors and there is also little penetration of the blood brain barrier in humans. Mild to Moderate toxicity can cause dizziness and headache but is not expected to cause an unsteady gait or paleness. He had a medical work up and no diagnosis or cause was known. His symptoms remain largely unchanged three days after removing the possible exposure. A re-challenge was not completed and would not be recommended at this time given his ongoing symptoms and unknown cause. A review of the previous cases and literature did not reveal other reported cases of similar nature.
