On 08-Oct-2019, a 7 year old, 5 pound, intact, female, Shih Tzu dog, in unknown condition, with known concomitant medical condition of fleas, was administered 1 dose of Hartz UltraGuard Rid Flea and Tick Shampoo with Oatmeal for Dogs (Phenothrin) via the topical route by the animal owner. Approximately five minutes post product administration, the dog exhibited disorientation and was brought to an emergency veterinary facility. Immediately upon arrival, CPR was performed but was unsuccessful and the dog died. The dog spent its time outdoors only, was unvaccinated and had no previous veterinary care. The pet owner was interested in pursuing a necropsy but it is unknown if one was completed. No further information was received in this case.

Assessment: The active ingredient in this product is present in very low concentration and has a wide margin of safety in dogs. Sudden onset of disorientation and death are not expected and do not fit the toxicological profile of the product. The owner reported the dogs lived outdoors exclusively, were unvaccinated, and had no prior veterinary care. Five pounds is a very light weight for an adult Shih tzu so it is possible the dog had underlying disease or had been under-nourished. Other causes including underlying severe flea anemia, infectious diseases, and cardiac disease should be considered. On 08-Oct-2019, a 6 year old, 10 pound, neutered, male, Shih Tzu dog, in unknown condition, with concomitant medical condition of fleas, was administered an unknown amount of Hartz UltraGuard Rid Flea and Tick Shampoo with Oatmeal for Dogs (Phenothrin) via the topical route by the animal owner. Approximately one minute post product administration, the dog exhibited disorientation and approximately five minutes later, he was lying in lateral recumbency. Approximately thirty minutes post product administration, the dog died at the pet owner's home. The dog was not evaluated by a veterinarian and no treatment had been performed. The dog spent its time outdoors only, was unvaccinated and had no previous veterinary care. The pet owner was interested in pursuing a necropsy but it is unknown if one was completed. No further information was received in this case.
