Five minutes after the product was applied on September 21, 2018, the owner bathed the cat two times with cat shampoo. About an hour later, the cat developed application site irritation. On the morning of September 22, 2018, the cat developed lethargy. In the evening on September 23, 2018, the cat developed fasciculations. The owner then took the cat to the emergency veterinarian and contacted the Animal Product Safety Service (APSS). The APSS veterinarian stated that cats do not tolerate permethrin and that there are some individual variations in sensitivity. The APSS veterinarian also stated that signs range from mild paresthesia, ataxia, and tremors to seizures and that signs can develop from almost immediately up to 72 hours post-exposure. The APSS assistant recommended the owner have the veterinarian call for information. In the early morning on September 24, 2018, the emergency veterinarian contacted the APSS to continue the case. The emergency veterinarian stated that midazolam (diazepam) was given for the fasciculations. The emergency veterinarian also stated that the cat had developed erythema at the application site. The APSS veterinarian recommended the emergency veterinarian bathe the cat (if the application site is still greasy), applying vitamin E (to the application site), provide fluid therapy, give methocarbamol (Methocarbamol (Robaxin-V, Fort Dodge, 100 milligrams per milliliter (mg/ml), 20 milliliter (ml), or 100 ml vial): cats 55 to 220 milligrams per kilogram (mg/kg) intravenous (IV) for severe tremor/seizure activity, give 1/2 rapidly (not exceeding 2 milliliters per minute (ml/min)), allow the animal to relax, and then give to effect (repeat to max 330 milligrams per kilogram per day (mg/kg/day)). If the cat's fasciculations are too severe for IV placement, try methocarbamol rectally first at 100 mg/kg.), provide nutritional support, provide symptomatic care, provide supportive care, and to call back with questions.
