On May 20, 2012, the dog found the container of the product sitting on a table outside and accidentally ingested some of the contents. Later that morning, the owner noticed that the dog was vomiting; a short time later, the owner noticed that the dog had seizures and tachycardia. After those signs appeared, the owner contacted the Animal Product Safety Service (APSS) to obtain help. The APSS veterinarian stated that methomyl is a carbamate insecticide with an extremely narrow margin of safety, and that toxic signs tend to develop rapidly and result in either death or somewhat rapid recovery. The APSS veterinarian recommended that the owner, who is a veterinarian, give 0.1 to 0.2 milligrams per kilogram of atropine for the cholinergic toxicity; 1/3 was to be given intravenously, and the rest could be administered intramuscularly or subcutaneously, and this could be repeated as necessary. The APSS veterinarian also recommended that the owner give oxygen, provide fluid therapy, call back, and decontaminate the environment to protect other dogs from exposure by using a Shop Vac on the lawn, putting water on the remaining granules of the product, and disposing of the vomitus.

Later on the morning of May 20, 2012 the owner called back to report that her dog had died several minutes ago. A necropsy was not available.
