On August 19, 2011 the owner applied one vial to the dog to treat for fleas. Later that day, the owner applied part of a second vial to the dog. On August 22, 2011 the owner applied the rest of the second vial and another full vial to the dog. On August 28, 2011 the owner applied a fourth vial to the dog.

On August 19, 2011, several hours after applying the product for the first time, the owner noticed the dog had pruritus. The owner continued to treat the dog with the product, and the dog continued to be pruritic. On August 29, 2011 the owner contacted the Animal Product Safety Service (APSS) to obtain help. The APSS veterinarian stated that permethrins and pyrethroids can cause a tingling sensation to the skin, as well as redness, dermal irritation, and pruritus. The APSS veterinarian stated that the signs often resolve after bathing and with topical applications of vitamin E and cool compresses, but it could take up to 24-72 hours for the signs to resolve fully. The APSS veterinarian stated that the signs could be more severe in this dog because the owner applied the product so frequently. The APSS technician recommended that the owner bathe the dog with liquid dish washing detergent (LDWD), open a capsule of vitamin E and apply it to where the dog was itching twice daily, apply a cold compress where the dog was itching for 5 minutes twice daily, and call back with questions. The APSS technician also provided information about how the owner could treat the environment to get rid of the fleas.
