On June 17, 2011, as soon as the first dose of the product was applied, the owner noticed the dog had pruritus. On June 18, 2011 the owner noticed the dog had erythema on her neck, abdomen, and tail. On June 20, 2011 the owner noticed the dog had alopecia around her neck and from the application site to the tip of the tail. On July 21, 2011, a few days after the product was applied for the second time, the owner noticed the dog's alopecia was worse. On August 1, 2011 the owner noticed the dog had dermal lesions. That day, the owner took the dog to her regular veterinarian for treatment, and the dog was prescribed Cefalexin. Later that evening, the dog was bathed with a dog shampoo. On August 3, 2011 the owner started applying a bag balm to the raw area of the dog's skin. On August 11, 2011 the owner contacted the Animal Product Safety Service (APSS) to obtain help. The APSS technician stated that the only expected signs would be mild dermal irritation. The APSS technician recommended that the owner bathe the dog with liquid dish washing detergent (LDWD) and consult with her regular veterinarian to find a topical product to apply to the dermal lesions and rash.

On August 16, 2011 an APSS technician called the owner to update the case. The owner stated that on August 16, 2011 she bathed the dog with LDWD and noticed that the dog's symptoms were milder than before. On October 26, 2011 an APSS technician called the owner to update the case, and the owner stated her elderly dog had died on September 11, 2011.
