6/17/08 Caller's spouse was working on a sprayer that had broken down that contained a blend of Prestige Herbicide parts A + B as well as another non-Dow AgroSciences agent. They suspect he likely got some of the diluted herbicide on his hands. There were two instances of him working on the malfunctioning application equipment over the course or two days (June 16 and June 17). On June 17 after again working on the application equipment, the husband developed a rapid heart rate and general weakness. He was seen in a local hospital and eventually admitted overnight for observation and treatment. It is not known what medications he received by he was send home with a medication to address his rapid heart rate. Husband was feeling fine by June 18.

The information contained in this report is based on self-reported statements provided to the registrant during telephone Interview(s). These self-reported descriptions of an incident have not been independently verified to be factually correct or complete descriptions of the incident. For that reason, information contained in this report does not and can not form the basis for a determination of whether the reported clinical effects are causally related to exposure to the product identified in the telephone interviews.
