History: Per CS Rep caller is upset and stating the product killed her dog, and wants taken off the shelf. Caller disconnected or was cutoff in transfer 09/27/07 Follow-Up Callback to caller. Explained I was with the adverse event line and want to make sure we have a complete report. States she was just walking out the door and could she call us back. Gave case # and # to call us directly. 09/28/07 Update Update forwarded by Hartz CR PHONE CONSULT DATE: Sept. 27th, 2007 "Kiera", a 6 years old dog, weighing 10 pounds, PRODUCT: Undetermined Hartz flea/tick product for dogs HISTORY: Owner reported that she sprayed a Hartz product on 9/25 (per CR form). She stated that the dog died and didn't give much information about signs. She also stated that she took the dog to a veterinarian but they couldn't do anything and the dog passed. I called the clinic that was listed on the CR form and spoke to receptionist who looked for the dog's record. She could not find a dog by that name. Nor could she find the client's name. She even put the client's phone number into her system and found nothing. I then spoke to Veterinarian who said he has not seen a dog as I described with possible Hartz toxicity in the past few days. He said he did remember that a lady that wasn't his client came into the clinic wanting to purchase flea control a few days ago. CONCLUSION: The owner did not take her pet to the above clinic as she said that she did. There is no way of investigating her complaints any further until she supplies correct information.
