4. Describe how the packaging failed and the surrounding circumstances, including a description of the potential injury or exposure.

Several pouches damaged during transit while in shipping box. Only the paper outside was damaged and the inner mylar layer remained intact. All packages were from the same lot. DDVP exposure could have occurred to any handler if the mylar layer had also failed. Customer was sent replacement product and they were notified to return or dispose of the damaged product. No record provided.

Prior to these complaints, this packaging had never been reported an issue. In response to the complaints, we are switching to a heavier mil. pouch/ package from a different supplier (the same pouches used to satisfy the US EPA's new CRP requirements). Once PMRA approves our amendment, we will begin using them for Canadian market Vaportape. Images of damaged pouches available by request.
