The reporters describe packaging failures involving a pesticide containing the active ingredient diquat. The reporters described leaking of product from totes (6), with failure reported to have occurred at the point of valve assembly and/or coupler. In four instances, the reporters noted the tote began to leak once coupled to a connector. In one instance, the reporter indicated that a broken o-ring at the discharge valve had occurred and approximated 2L of product leaked. No exposure or environmental injury was reported.

Annual cleaning and inspections of the totes (including seals and o-rings) and regular scheduled replacements (o-rings) for these returnable 450L totes instituted by the registrant as preventative maintenance to ensure quality standards achieved. Pressure testing of the totes is also conducted every 30 months (by [Department] [Country], according to mandated process).
