The reporters describe a packaging failure involving a pesticide containing the active ingredient pinoxaden. The reporters described leaking of product from 400L totes (2) with failure reported to have occurred at the point of valve assembly and/or coupler and approximations of 50 -200L of product leakage reported. No exposure or environmental injury was reported.

Annual cleaning and inspections of the totes (valves, seals, o-rings) and regular scheduled replacements (o-rings) for returnable totes conducted as preventative maintenance to ensure quality standards achieved.
