Subform VI: Packaging Failure

4. Describe how the packaging failed and the surrounding circumstances, including a description of the potential injury or exposure.

It is believed that the load on the transport truck was cross-braced at the front about 8ft from the front wall of the trailer. No dunnage was placed between the front wall and the totes stacked 8 ft back. When the load arrived, all totes were jammed up against the front of the trailer and the cross brace was sandwiched between the totes and the front wall. It had been ripped from the brace railing on both sides. It would seem that the driver hit brakes hard during the trip causing the totes (all of them) to slide forward with such force that it broke the cross brace free and lodged it (suspended off the floor) to the wall. The force also is believed to have ruptured the tote causing the leak.

Warehouse staff told the driver that something was leaking. Driver acknowledged that he was aware of the issue but didn't apparently stop/call anyone and continued to [Company] [City]. [Company] called [Company] who showed up to deal with the cleanup.
