The reporters described packaging failure involving a product containing the active ingredient diquat. The reporters described leaking of product from totes (6), with failure reported to have occurred at the point of valve assembly and/or coupler. In three instances, a dislodged or lack of o-ring at the valve was reported. In the other three instances, leaking was observed only when the valve was open but no additional details/observations were reported regarding the state of packaging or valve assembly. The amount of leakage was not provided in all instances. When reported, product leakage approximations ranged from 50 - 160L. No exposure reported.

Annual cleaning and inspections of the totes (including seals and o-rings) and regular scheduled replacements (o-rings) for these returnable 450L totes instituted by the registrant as preventative maintenance to ensure quality standards achieved. Pressure testing of the totes is also conducted every 30 months (by Transport Canada, according to mandated process). Dislodging/unseating of o-rings is uncommon but may have been a factor at the end user level. Totes have been requested back for investigation.
