(eg. description of the frequency and severity of the symptoms
On approximately 15-Apr-2021, a 2 year old, 50 pound, neutered, female, Labrador Retriever dog, in unknown condition, with no known concomitant medical conditions, was administered an unknown amount of Hartz UltraGuard Rid Flea and Tick Shampoo for Dogs 18oz (Phenothrin (Sumithrin)) via the topical route by the animal owner. The owner had routinely bathed the dog using the product 1-2 times per month for an unspecified length of time without adversity. On 22-Apr-2021, the dog died suddenly. The dog had not been evaluated by a veterinarian, no treatment had been performed and a necropsy was not performed. No further information was received in this case.
Death
Assessment: The product contains a pyrethroid insecticide in very low concentration giving it a wide margin of safety in dogs. Sudden death is not expected, especially one week after use. Without a diagnostics including necropsy the cause of death remains unknown, though underlying cardiac disease, toxin exposure, unknown trauma, and other causes should be considered. The information contained in this report is based on self-reported statements provided to the registrant during telephone interview(s). These self-reported descriptions of an incident have not been independently verified to be factually correct or complete descriptions of the incident. For that reason, information contained in this report does not and cannot form the basis for a determination of whether the reported clinical effects are causally related to exposure to the product identified in the telephone interviews.
