Subform VI: Packaging Failure

4. Describe how the packaging failed and the surrounding circumstances, including a description of the potential injury or exposure.

A box on the pallet had tipped during transportation with X and caused 4 jugs to leak. Foil seals separated slighlty from the containers when they fell from the pallet. The cases were not shrink wrapped on the pallet. The box is soaked in chemical and 4 jugs with broken seals and 4 jugs that are covered in Aim EC Herbicide. Update from email from FMC including report from Rep. X dated 4/16/2021. Aim Jug showed up at X with foil peeled back and lost about 400ml of Aim. It stained 3 other cases and had another jug that had a slice on it and was leaking as well. Action to Resolve: Replaced their 2 jugs of damaged with my demo product, will pick up damage jugs from X next week.

Update from email from FMC including report from Rep. X dated 4/16/2021. Aim Jug showed up at X with foil peeled back and lost about 400ml of Aim. It stained 3 other cases and had another jug that had a slice on it and was leaking as well. Action to Resolve: Replaced their 2 jugs of damaged with my demo product, will pick up damage jugs from X next week.
