The following was reported: Producer (name) purchased Nufarm's Tralkoxydim herbicide for 617 acres through x in (city).This product was applied on barley at 6 leaf stage and covered about 500 acres of barley. It was mixed with Corteva's Pixarro.

4. Describe how the packaging failed and the surrounding circumstances, including a description of the potential injury or exposure.

The following was reported: When they mixed it up to spray (it was mixed with Corteva's Pixarro), they noticed some of the cases had Tralkoxydim labels but the 8L jug inside the case were labeled with the product Koril 235 Liquid Herbicide Reg # 25341. The producer applied this product to barley not realizing this could be Tralkoxydim or Koril as he was reading the labeling on the cases. Beside the wrong label on the jugs and in a Tralkoxydim case, the producer could not clean his sprayer out due to a pastey substance found in the booms, tank and filters. It took the producer over eight hours to clean out the sprayer along with various tank cleaners and pressure washer. We do not know if the product was just wrongly labeled or there was the wrong product in the jugs and this could have caused the pastey substance in the spray tank, boom and screens.
