The patient is a male neutered 7 year old 8.6 kg shih Tzu. The patient was recently diagnosed as a diabetic (Aug 2018) but is not currently treated with insulin. The patient was groomed and shaved on August 16th and had a strong perfume smell. On August 17th, the pet owner applied 1 vial of K9 Advantix II medium dog.

The same day the patient was very itchy and rolling around, scratching at his flanks and his ears. This patient has had K9 Advantix II in the past without previous history of reaction. The outcome is unknown.

B - Possible - Though would not expect generalized skin disorders after topical application, reported itching and rolling around may reflect paresthesia caused by permethrin. Scratching is not expected to be localized at flanks and ears but rather be linked to the application site. However, may be a consequence of the uncomfortable feeling caused by the applied product. Considering consistent time to onset, a product connection is deemed to be possible.
