8/1/2016 Caller is a pest control operator reporting that 0.01 percent concentration product was applied as a crevice treatment for spiders in a condominium complex 7 days ago. The patient was a young female puppy who had recently been spayed. The puppy was owned by the brother of a new tenant in the complex. The puppy was not left unattended in the complex, and did not come into contact with the product. Caller does not have full information regarding the patient and the signs shown. The puppy became ill 2 days after the product was applied. The puppy was taken to a veterinarian, but died 2 days later on 7/29/2016. The veterinarian performed a necropsy, and reported that there were holes in the puppy's heart and other organs. Tissue samples are being sent to a university for further examination.
