(eg. description of the frequency and severity of the symptoms
3/9/2016 2:44:24 PM Caller placed two blocks of product in a bait station in her basement about two weeks ago (approximately 24Feb2016). No blocks were missing from the bait station and the caller does not suspect the dog directly ingested the product, regardless she is concerned that there may have been an exposure to the product that she cannot quite account for which harmed her dog. On the evening of 02/26/2016 the dog began shaking when she inhaled. The following morning, 02/27/2016, the dog developed vomiting and seemed warm to the touch. The dog was examined by an emergency veterinarian on 2/28/15 for a protruding tongue. The veterinarians stated the dog was dehydrated and febrile. She administered fluids and unspecified injectable medications; the dog was then discharged with unspecified oral medications. On 02/29/2016 caller took dog back to the veterinarian as the symptoms persisted; the dog was hospitalized on intravenous fluids. Later that day, the dog's symptoms worsened and it started vocalizing; the dog died overnight at the hospital.
