9/18/2012 Caller is a company representative calling to relay information from a pest control officer regarding a companion animal fatality. The pest control officer applied the product around the perimeter of a home. The homeowner reports that his dog became ill at some unspecified point after application, and died a few days later. It is not known how this dog would have been exposed to the pesticide. No known symptoms reported. Attempted call to the homeowner. A message was left requesting follow up information. Owner did not follow-up as of Oct. 5, 2012.
