On July 1, 2012 and again on July 2, 2012 the owner sprayed the product onto a sheet on the dog's bed, allowed it to dry, and crumpled the sheet before putting it back on the dog's bed.

Sometime between July 2 and July 4, 2012 the owner observed that the dog had died. On July 8, 2012 the owner contacted the Animal Poison Control Center (APSS) to obtain help. The APSS veterinarian stated that given the exposure the owner described, no signs would be expected at all. The APSS veterinarian recommended that the owner have his veterinarian call for information and to provide additional information. A necropsy was not available because the dog's body had been cremated.

The APSS veterinarian stated that the substance was considered to have an unknown likelihood of causing the clinical situation. On the afternoon of July 8, 2012 the owner called the APSS to obtain contact information for the product's manufacturer. The APSS veterinarian gave the owner the phone number of the product's manufacturer.
