1/28/2010 A consumer incident report was forwarded for documentation stating that a consumer developed symptoms after applying the product. 1/29/2010 Callback attempted to the original caller to document the events. A message was left requesting follow up information. 2/1/2010 Caller is returning the previous message. Caller reports using the product 9 months ago. Caller states that the spray nozzle may have been defective, and the product sprayed out of the container too liberally during application. The product pooled on her scalp, and caller felt a burning sensation on her scalp. Caller showered following application. The burning sensation turned to itching, and resolved within several days. Caller has remained asymptomatic.
