1-16365903: The reporter called on 7/7/08 to report the death of her 6 week old puppy after the application of a topical flea and tick collar containing the active ingredient Tetrachlorvinphos. According to the reporter, the collar had been applied on 6/29/08. Within 2-3 days (7/2 or 3), the puppy had begun salivating and had appeared ┐┐┐lifeless┐┐┐. The collar had been removed on 7/5, and the puppy had passed away later that same day. The company sponsored cholinesterase testing program was discussed but the reporter declined.
