On 07 Mar 2023, a pet owner contacted Elanco to report a possible adverse reaction regarding Advantage II Large Cat (Imidacloprid, Pyriproxyfen) regarding a cat. On 07 Mar 2023, the pet owner administered administered 1 tube of Advantage II Large Cat (Imidacloprid, Pyriproxyfen) topically. The product was not used correctly as it was an overdose. The cat licked the product after application.

On 07 Mar 2023, 15 minutes post administration of the Advantage II, a 7-year-old, 3.67 kg, female neutered, Siamese cat in unknown condition with a history of constipation, issues with hairballs, recent diet change, recent change in living conditions, and vomiting, developed foaming at the mouth and lethargy. On 08-Mar-2023 the cat vomited in the night and did not have any bowel movements. The owner treated cat with a dose of lactulose previously prescribed by DVM for constipation and hairballs. The owner commented cat was not back to her regular routine from a recent change in living conditions and diet. On 09-Mar-2023 cat vomited at 9 am and was anorexic. The pet owner dropped the cat off at the veterinary clinic to be examined. The veterinary examination on 9 Mar 2023 revealed the cat was slightly dehydrated. The owner reported the cat's vomitus was pink tinged. A rectal examination revealed some hard stool which the cat defecated which was then followed by soft stool and frank blood, described as hematochezia in the records. The cat was sedated to obtain bloodwork, which revealed elevated glucose 15.6 mmol/L (4.0-9.7 mmol/L), elevated Blood Urea Nitrogen (BUN) 16.1 mmol/L (5.7-13.2), elevated anion gap 29 (12-25), low total protein 56 g/L (63-88) with low globulins 25 g/L (30-59), and low cholesterol 2.2 mmol/L (2.4-7.9). Feline pancreatic lipase immunoreactivity (spec fPL) was elevated at 4.8ug/L (0-3.5) and a diagnosis of pancreatitis was made. Fructosamine and complete blood count were normal. The cat was treated with meloxicam, ampicillin, cefazolin, maropitant, famotidine and subcutaneous fluids by injection, then sent home with oral lactulose, sucralfate and famotidine. On 10 Mar 2023 the owner reported the vomiting and diarrhoea had stopped and the cat was eating, drinking and had normal energy. On 11-Mar-2023 Elanco contacted clinic. The veterinary clinic reported cat recovered, is no longer vomiting and has returned to normal. Abnormal test results had not been rechecked.
