Since an unknown date in approximately Sep 2021, an 18 year old, 10 pound, neutered, male, Dachshund (Unspecified) dog, in unknown condition, with concomitant medical condition of flea dirt and blindness in one eye, was administered an unknown amount of Hartz Ultraguard Pro Triple Active Flea and Tick Shampoo for Dogs (Methoprene / Phenothrin (Sumithrin) / Pyriproxifen (Nylar)) via the topical route by the animal owner. The dog continued to be bathed weekly with the product and the amount administered by the animal owner was more than the recommended dose, though an exact amount could not be specified. The dog would occasionally lick the bath water. Immediately after each bath, the dog developed disorientation, confusion and lethargy and the symptoms resolved within approximately thirty minutes. After the first time use, on an unknown date, the dog developed weight loss. On an unknown date in approximately Oct-2021, the dog developed blindness in his other eye. The dog was last bathed using the product on approximately 01-Jan-2022. On 13-Jan-2022 the dog died of unknown causes and was buried. The dog had not been evaluated by a veterinarian and no necropsy was performed. Another dog in the household was also bathed weekly with this product and the amount administered was more than the recommended dose. After each bath, the dog exhibited teeth chattering then recovered approximately five minutes later (coded as minor severity).

Assessment: This product contains a pyrethroid insecticide and two insect growth regulators in low-concentration giving it a very wide margin of safety in dogs. Even if the product was ingested directly during bathing only minor signs of GI upset would be expected. Progressive weight loss, loss of vision, and death are not expected and do not fit the toxicological profile of the product. The transient confusion and lethargy reported after bathing may have been related to a paresthesia reaction (tingling sensation on the skin) that may occur after use of some pyrethroids. Given the signs reported and the advanced age of the dog, other causes including neoplasia (cancer), underlying metabolic disease, and cardiac disease should also be considered. Without a necropsy, the cause of death remains unknown and the relationship to product use is inconclusive.
