On May 1st around 2pm the Univar warehouse in (city, province) was unloading a 1000L tote of The Rack Glyphosate, PCP 30442. There were 16 totes on the trailer. 15 out of 16 were unloaded without problems. On one tote the forklift forks slipped in between the pallet boards and punctured the bladder. The spill drained into the loading dock, a bit in the truck, the dock itself and a bit on the warehouse floor. The product was contained to the loading dock area. Univar staff then proceeded to pump out the loading dock area into 2 empty totes and also used 14 bags of Celatom. This was disposed of into a spare tote they had cut a hole in top of. They also used 2 salvage drums for what was cleaned up from the warehouse and truck trailer floor. Celatom was used in the trailer and warehouse floor as well. Univar plans to dismantle the pallet, the bag liner, and soiled cardboard in a salvage drum to be disposed of as well as the other waste drums from this incident. Loading and unloading the totes they have noticed they are on an angle and to offset this the fork angle must be adjusted. This is partly due the placement of the tote strapping. This strapping placement allows the forks to slip over it at an even steeper angle. The pallet quality appears to be constructed of lumber that is lighter quality or soft wood than what other companies use. It is recommended that we use a better quality of pallet, and the location of the strapping is moved. The photo showing number 1 & 2 would be the recommended placement for the strapping to further prevent this from happening. The 15 remaining totes were shipped to a customer today. They checked the strapping on the remaining pallets and it was located in the same place. To prevent further incident the staff at the (name) warehouse doubled up the pallets using extra ones they had. Recommendations: Strapping placement be relocated Hard wood pallet be used Plywood on top of pallet underneath tote for extra protection To be proactive (name) spoke with Manitoba Conservation to advise them what had happened. It was not a product required by the dangerous goods regulations to be reported. Manitoba Conservation was pleased with Univar's response plan and no further action will be required.
