On July 9, 2012 the owner noticed that her puppy accidentally ingested some of the bait product. A short time later that day, the owner observed that the puppy was symptomatic. A short time after observing the signs, the owner contacted the Animal Product Safety Service (APSS) to obtain help. The APSS veterinarian stated that methomyl is a very potent carbamate insecticide and that any exposure would be considered to be of concern. The APSS assistant recommended that the owner take the puppy to the veterinarian immediately, have the veterinarian call for information, and call back with questions.

The APSS veterinarian stated that the substance was considered to have a medium likelihood of causing the clinical situation. On July 13, 2012 an APSS assistant called the owner to update the case. The owner stated that the puppy died shortly after the initial consultation with the APSS on July 9, 2012 she was not able to get the puppy to the veterinarian.
