On May 15, 2012 the owner noticed that the puppy had an oral hemorrhage. Less than an hour later, the owner observed that the puppy had died. Later that day, the owner contacted the Animal Product Safety Service (APSS) to obtain help. The APSS veterinarian stated that mild nausea, vomiting, and diarrhea could be seen if the shampoo was ingested and that no signs were expected with dermal use of the product. The APSS assistant recommended that the owner call back with questions. A necropsy was not available because the owner did not have the puppy's body anymore.

The APSS veterinarian stated that the substance was not related to the clinical situation. A follow up was not performed, because additional information was not expected. Important label information was ignored which is: Do not use on dogs under 12 weeks.
