4. Describe how the packaging failed and the surrounding circumstances, including a description of the potential injury or exposure.
A retail location noticed a small amount of liquid had soiled a few other cases. It appeared that a small pinhole at the bottom of one jug of MCPA caused the product to leak out and wet a few surrounding boxes. The cases were all still shrinkwrapped on the skid. No exposure occured, and was cleaned up and contained easily.
